I finally got around to upgrading my PICStart+ firmware. I tried your new 
patches for programming an 18F452. They work!

I verified that program memory and the configuration fuses can be written.  
I was a little confused when erase failed to work, but determined that
this was not a problem. (The write operation will re-program each location
anyways). With MPLAB, I noticed that the erase operation is very brief
(less than a couple of seconds). With picp, it takes about a minute and
then it reports that it failed.

I have not tested code protection, data memory program, or ID location 
programming.
