I've implemented it exactly how Windows does it.  We should
patch to check for .ttf and insert 'b' before, or at least
specify that the extension should never be given.

If you do this, you'll need to implement it as discussed, by passing
the attribute down to the font sub-driver.  In order to get
the bold value, we'd need to pass the PMWLOGFONT
struct all the way down, as well, instead of stopping at
GdCreateFont().
